Application Process
Applying to join the network of patients is simple. Just fill out the Application Form and your responses will be viewed by our staff who will reach out with further questions if necessary.
Whether you are a patient without a provider or a patient not satisfied with the relationship with your provider, NEM's goal is to provide a space for grassroot action for patients and providers to form a long-term partnership.
Once connected, the patient-provider partnership will continue as long as both are committed to the mutual goal of offering personalized, relationship-oriented care.
We know that finding the right provider can be difficult, but our priority is to create a community focused on the needs of the patients by matching them with providers who meet their immediate needs and help them achieve long-term healthcare needs.
